About The Role

We are seeking dedicated and experienced Patient Flow and Access Discharge Concierge Coordinators. This role is crucial in ensuring that our funding allocation targets key measures, as required by the Ministry of Health (MOH). This casual role will be based at Gosford Hospital, covering a 7‑day roster that includes 8‑hour shifts from Monday to Friday and 6‑hour shifts on Saturday and Sunday.

The Patient Flow and Access Clinical Nurse Coordinator will work in collaboration with the Patient Flow Manager (PFM) / After Hours Nurse Manager (AHNM) to identify, monitor, communicate and elevate if required any issues related to delays in the patient journey across the facility. The role provides comprehensive support to Nurse Unit Managers in embedding best‑practice discharge processes, assists with discharge preparation, generates and distributes patient flow reports and assists with weekend support to After‑Hours Nurse Managers (AHNM) and Patient Flow Teams.

The Patient Flow and Access Clinical Nurse Coordinator plays a pivotal role in delivering specialised care coordination, enhancing communication, and improving both patient and carer experiences. Working collaboratively with clinical multidisciplinary teams, this role leads efforts to improve the discharge process, offering expert guidance to ensure efficient hospital operations and high‑quality patient care. CCLHD is looking for someone who is passionate about improving patient flow and discharge processes.

About You
  • Current registration with AHPRA as a Registered Nurse (Division 1) with relevant post‑registration qualifications and at least 3 years’ experience working in the clinical area of their specified post‑graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
  • Demonstrated experience or willing to work towards improvement in continuous clinical practice, project management, and/or change management.
  • Demonstrated clinical leadership skills to act as a resource and mentor to other health care professionals, develop innovative workplace solutions, and empower stakeholders.
  • Proven ability to collect and analyse quantitative and qualitative data to evaluate projects and prepare high‑level reports.
